• Athlete Status* -- Indicate if you are a OLY or PLY athlete
• If OLY/PLY Athlete -- Provide Games Info (Games Year/s, Sport/s Competed in, Country Represented; if not leave blank
• If Other Games' Stakeholder Group -- See diagram above to select group that best identifies you (local official, Games volunteer, legacy site,  or local private citizen, business, media, etc.)
• Select Host City -- Pick Host City of the Games where you worked, lived, competed, volunteered,...or just loved! Note: You can join multiple Host City Teams           
• Other/Misc Info -- Optional: such as additional contact information, interests, etc.
• Interest in helping OLY PLY CITIES -- Optional: Indicate interest in serving on OPC Advisory Board and/or Regional Organizing Committee
• Interest in participating in a global Zoom Call to learn more about OLY PLY CITY
